AST Fall Protecton

I am hoping to get information on the various ways companies are implementing fall protection when the technicians are working on top of AST's that do not have walkways. What are people having their techs/ pipe-fitters tie off to? Because every tank has different piping configurations I am hoping to learn new alternatives.

-Temporary scaffolding is very common and is the safest
-Using one of the pipe nipples or risers on top of the tank, you can construct a tie-off "T" (not common)
-Urge the client to install a better safety ladder if routine maintenance is being performed
